This is the first inpatient admission of this 35 year old, single, African american male who has a college degree and is employed as a manager for a major department store. He was admitted due to symptoms of major depression with possible psychotic features.
FAMILY HISTORY
localise was born and raised in Chicago. His parents are originally from tungsten African mainly Ghana. They are well educated and right off live in Texas. He has two sisters that live in Chicago. He says he is a Baptist however seldom goes to church. He is currently living with his girlfriend. He has very few friends and enjoy to be alone. He enjoys fishing and occasionally socialize. His dad is an torrent and Mark is showing signs of it also.
MENTAL STATUS EXAM
The result of the moral status exam reveal he is alert and attentive . He was well dressed and groomed. His clothes were neat but he had a smell of alcohol. Eye contact was say and he seldom looked away. His posture was good. Vocabulary and grammar shows he has just about college education. His tone of voice was soft spoken. His hands were trembling and folded as he spoke. The exam was 30 minutes.
He was cooperative and answered each question. He thought before answering and seemed organized. He did non interact with my staff when he walked in and seemed shy. There was no evidence of paranoia in his thought.
SUMMARY/RECOMMENDATION
The result show a history of alcoholic ab wasting disease. He lacks self esteem and has a problem with trusting women. He shows no sign of bellicose behavior. There is no physical risk to others or himself. I would recommend group therapy so he can study to socialize and mix with people. I will need to course his trust and encourage him to socialize.
